There is no cost to join Diabetes Care Club... and the process is quick and simple.

• Call us toll free at 1-800-840-7711. During our initial phone conversation, one of our skilled Patient Care Representatives will determine which Diabetes Care Club products are appropriate based on your needs and how many times you test your blood sugar a day. We'll verify these needs with your physician and obtain documentation of your diabetic testing supplies requirements, which normally only takes a couple days.
  
  
• Once we receive the required documentation from your doctor, we'll ship your first order. No pre-payment is ever required. An Assignment of Benefits form will be included in your first order, which you'll need to sign and return to us. This form allows us to bill Medicare on your behalf. From this point on, the process is automatic!
  
  
• We then bill Medicare and/or your insurance company directly. Our electronic claim submission is verified by Medicare and/or your insurance company and the allowable payment(s) are made directly to Diabetes Care Club. Once we determine how much of your shipment is covered by Medicare (80% once your annual deductible is met) and/or your insurance company (the remaining 20%), we bill you only for those charges not paid by Medicare and/or your insurance.
  
  
• Your assigned Patient Care Representative will call you before you run out of supplies and ask if you need a refill. You have total control - we will only ship you what you and your doctor order. It's that easy! Receive an uninterrupted flow of the diabetes testing supplies you need, at little or no cost to you... with little or no paperwork.
