Information We Collect

You may browse our product offerings and other portions of our Website without providing any Personal Information. However, if you wish to purchase a product through the Website or utilize the social media features of the Website, which are available under the "DCC Connect" portion of the Website, we will ask you to provide Personal Information (as defined below). The DCC Connect portion of the Website also enables Members to voluntarily share their Health Information (as defined below) related to diabetes and diabetes treatment by learning about others with diabetes. The types of Information we collect include: 


Personal Information - Personal Information is information that uniquely identifies Members, such as name, address, contact information (e.g., email address, phone number), credit card information, photo, biography, date of birth, and gender.

Health Information - Health Information is information Members voluntarily upload about their health status, as part of their user profile on the DCC Connect portion of the Website. Examples of Health Information are details about conditions or diseases that Members have, including the first notice of symptoms, date of diagnosis, co-existing conditions, family history, quality of life effects, list and severity of symptoms, timeline or progression of disease, results of laboratory tests, details of treatments tried (e.g., dates, dosages, measures of effectiveness in terms of quality of life), and other factors that could possibly be related to or associated with the diabetes or other related conditions (e.g., environmental factors, personal traits, ethnicity, and other factors as defined by members). Members can enter as much or as little Health Information as they like. To provide the highest quality features and benefits for a great Member experience, we may review Member’s Health Information internally, as needed. 

Resource Information - Resource Information is information Members post about treatments, doctors, studies, things to read, or support resources, or reviews about their experience with the posted resources. Members can choose to post resources and/or reviews anonymously or with a screen name linked to their profile. Posted resources and reviews will be visible to all Members and visitors to the site, to help people learn from each other’s experiences. 

Site Usage Information - Site usage information consists of log files and cookies, which are required to use the features located on the DCC Connect portion of the Website. Log files are automatically collected and include information about browser/operating system types, movement to/from and around the Website, time spent, and IP address/ISP. Log files may used on an aggregate level to analyze trends and understand Member demographics and behavior on the Website, and are only linked to Personal Information for specific cases to resolve Member issues. Cookies are small data files that do not contain Personal Information. Cookies are sent to a browser from a visited website and stored on a Member's computer. We may use session cookies that persist only while the browser is open and at the Website, and persistent cookies, that remain in the browser's cookie file even after leaving the Website. These cookies may enhance a Member's experience; with easier navigation, improved security of Personal Information, and additional functionality. We also may use Site Usage Information to improve our services for our Members. Although it is non-personal, Site Usage Information is linked to Personal Information if a Member is logged into our Website.

Uses of Personal and Health Information

We use Personal and Health Information we collect to process your requests, authenticate Website visits/usage, inform Members of relevant services and products, fulfill product orders, maintain and develop our Website, and communicate with you. You always will be given the opportunity to unsubscribe from messages in any such messages we send you. Members will also be able to see the parts of each other’s' profiles that are publicly shared. 

When you make a purchase on our Website, we may need to provide your Personal Information, including information such as your shipping address, billing information, telephone number, and credit card information, to third parties, including product manufacturers and shipping companies, as necessary solely to complete the transaction.

We may use, open up or disclose any Information you provide for research purposes, in an anonymous and aggregate way, unless you explicitly inform us not to use or disclose your Information for research purposes. 

We protect all patient data through rigorous security measures. We will not disclose, rent, sell or share any Personal Information or other personally identifiable Information to third parties for marketing purposes. PERSONAL AND HEALTH INFORMATION THAT YOU CHOOSE TO PROVIDE VIA THE DCC CONNECT PORTAL OF THE WEBSITE IS NOT CONSIDERED “PROTECTED HEALTH INFORMATION” AND IS, THEREFORE, NOT SUBJECT TO HIPAA OR ANY OTHER FEDERAL OR STATE LAW GOVERNING THE PROTECTION OF INDIVIDUALLY IDENTIFIABLE HEALTH INFORMATION. DCC WILL NOT DISCLOSE OR MERGE PERSONAL AND HEALTH INFORMATION OBTAINED VIA THE WEBSITE FOR PURPOSES OF PROVIDING YOU WITH PRODUCT OR SUPPLIES WITH INFORMATION SHARED BY YOU VIA THE DCC CONNECT PORTAL OF THE WEBSITE, UNLESS WE HAVE YOUR PRIOR WRITTEN AUTHORIZATION TO DO.

DCC Connect: Community/Chat Rooms/Message Boards

The DCC Connect portal of our Website includes online forums, blogs, chat rooms, message boards, and other interactive technologies that allow comments, messages, emails, and postings to be associated with a Member’s name or email address. If you choose to make your Information publicly available, other Members will be able to see your Information and learn directly from your experience. However, it is your choice and right to keep as much Information confidential as you like. Members can choose to have their Information freely available, only used for research purposes, visible only by approved friends, or kept completely private. If you choose to make your Information publicly available, we cannot control what use third parties may make of this Personal Information. Please consider this before you post.

Privacy and Security

Members can change privacy settings and edit their Personal Information at any time by selecting the “Edit/Update” button found within the “My Account” section of the Website, but completely public profiles may be indexed by Internet search engines such as Google. We encourage sharing as much as you are comfortable with to maximize the benefit to everyone in the community.

Information that you provide via the DCC Website for purposes of obtaining product or supplies is maintained separately from Information you may disclose on the DCC Connect portal of our Website. If you choose to delete your account, all Personal and Health Information is removed from our Website databases. However, any Personal or Health Information you have authorized to be used or disclosed for research before requesting deletion will not be removed from research studies that are in progress or completed. Any Personal or Health Information you have disclosed on the DCC Connect portion of our Website is not subject to the privacy and security obligations set forth in this paragraph and may not be removed.

Protecting your Personal Information is a shared responsibility, so we encourage our Members to keep their passwords and other sensitive Information private and secure. All orders are transmitted over secure Internet connections using SSL (Secure Sockets Layer) encryption technology. Although we use reasonable efforts to safeguard the security of your Personal Information, transmissions made on or through the Internet are vulnerable to attack and cannot be guaranteed to be secure. Risks of Sharing Personal and Health Information on the Website

We have no way of guaranteeing the validity of the Information that Members may submit and make publicly available on the Website, nor can we authenticate the identity of any Member, so any Information you review is at your own risk and should be discussed with your doctor or alternative medical provider.

Children Under 13

We do not knowingly collect or solicit Personal Information from anyone under the age of 13.
