• Insurer's name and phone number
• Your policy number and subscriber number

Your secondary insurance coverage:

• Insurer's name and phone number
• Your policy number and subscriber number

We will contact your insurance carrier(s) or HMO to determine what portion of your diabetes testing supplies is covered - and how Diabetes Care Club can best meet your needs. In most cases, we're able to supply all the diabetes testing supplies you need.
